1095-A forms are sent to consumers by Georgia Access via their selected delivery preference in January. Dental and catastrophic plans do not receive a 1095-A.
If electronic delivery has been selected, consumers can log into the Georgia Access portal and access an electronic copy in their secured inbox. If you’ve selected postal mail, you will receive the form by January 31st.
If you are using one of the EDE platforms, access instructions for your account may differ so you should contact them directly to request your 1095A.
If you need to request a copy of your 1095-A to be mailed to you and you are not affiliated with an EDE, please call the Contact Center and follow the prompts to request a new copy.
