When you apply for Georgia Access coverage through a Special Enrollment Period (SEP), you may be asked to provide documents to confirm the Qualifying Life Event (QLE) that makes you eligible. See below for commonly reported QLEs that require documentation, what documents you can submit, and how to submit them:
Confirm a permanent move into, or within, Georgia Access
Examples of documents you can submit include:
- Postal Service change of address confirmation letter, including your mail forwarding date and the address mail will be forwarded to.
- Mortgage or rental document, including a mortgage deed that says the owner uses the property as a primary residence, or a rental or lease agreement that shows a start date at your new address.
- Bills or financial statements that show a change of address or newly started services at your new address, like mail from a financial institution or a bank statement; or an internet, cable, phone, or other public utility (like gas or water) bill or service communication.
- Letter from a government organization on official letterhead or stationery, that shows a change of address to your new address; like a Social Security statement, a notice from Division of Family and Children Services for SNAP (food stamps) or TANF (cash assistance) benefits, mail from the Department of Driver Services (DDS), mail from the Internal Revenue Service (IRS), mail from the Low Income Home Energy Assistance Program (LIHEAP), or a voter registration card.
- Insurance document, like a homeowner’s or renter’s policy or statement showing your current address.
- Document showing you moved from outside the U.S., like an Arrival/Departure Record (I-94/I-94A) (in a foreign passport or separately) that shows your date of entry into the U.S., or a copy of a passport with an admission stamp showing your date of entry into the U.S.
NOTE: When a qualified individual or enrollee, or their dependents, gain access to new Qualified Health Plans (QHP) following a permanent move, they must demonstrate (provide document validation) that they had active health insurance / minimum essential coverage (MEC) for at least one (1) day during the last sixty (60) days preceding the move.
f you moved to the U.S. from abroad or from a U.S. territory, you need to prove that you lived outside the U.S. When you send documents proving your move and move date, make sure you include a document showing that your old address was outside the U.S.
If you’re homeless or in transitional housing, you can submit a reference letter from a person in your state who can confirm that you live in the area permanently. This could be a friend, family member or caseworker. This person must also prove their own residency by including one of the documents listed above.
Confirm a marriage
Examples of documents you may submit include:
- Marriage certificate showing who got married and the date of the marriage.
- Marriage license showing who got married and the date of the marriage.
- Official public record of marriage showing who got married and the date of the marriage.
- Marriage affidavit or affidavit of support that’s signed and dated by the person who officiated the marriage or the official witness of the marriage.
- A religious document showing who got married and the date of the marriage.
Gaining an eligible immigration status
Examples of documents you may submit include:
- Birth certificate
- Certification of report of birth
- Certification of citizenship or naturalization
- Evidence of U.S. Civil Services employment before June 1, 1976
- Federal or state census record
- Final Adoption decree
- Machine-readable immigrant visa on unexpired foreign passport
- Medical records
- Military record showing U.S. place of birth
- Minnesota enhanced driver’s license or enhanced identification card
- Northern Marianas ID card
- Official religious records
- PASS card
- Report of birth abroad of a U.S. citizen
- School records
- Section 1-1 of the Child Citizenship Act of 2000 documentation
- Temporary I-551 Stamp
- Tribal Affiliation card or document
- Unexpired foreign passport
- S. citizen ID card
- S. passport
- DS2019 (certification of eligibility for exchange visitor status)
- I-20 (certification of eligibility for non-immigrant student status)
- I-94 (arrival/departure record)
- I-94 (arrival/departure record) in unexpired foreign passport
- I-327 (reentry permit)
- I-551 (permanent resident card)
- I-571 (refugee travel document)
- I-688 (temporary resident card)
- I-688A (employment authorization card)
- I-688B (employment authorization document)
- I-766 (employment authorization card)
- Other documents generally accepted as legal proof of citizenship or immigration status such as:
- Administrative order staying removal issued by the Department of Homeland Security
- Document indicating withholding of removal
- Form I-360 prima facie determinations
- Office of Refugee Resettlement
Documents must show you gained lawfully present status within the past 60 days. These documents must include your name, immigration status and date on which citizenship or lawful presence was gained.
Gaining eligibility for Health Reimbursement Accounts (HRA) such as an Individual Coverage HRA (ICHRA) or Qualified Small Employer HRA (QSEHRA)
You must send a notice from your employer showing when your individual coverage HRA or QSEHRA can begin.

- Once you’re logged in, you will see an ‘upload documents’ link on your Dashboard. Click this link and follow the steps on the page to upload documentation to help us verify whether your life event qualifies you for a Special Enrollment Period.
- After you submit your documentation, Georgia Access will review and determine whether your life event qualifies for a Special Enrollment Period. Please check back often for the latest updates to your account and any next steps required.

- Make sure to write your name and Application ID, which would be included on your eligibility notice, on all your documents. You should keep the original documents for your records.
- Include this notice’s cover sheet with the barcode when mailing your documentation.
